1. Introduction
Welcome to Elevate Mental Health Services. By accessing or using our telehealth services, website, or scheduling an appointment, you agree to these Terms of Service. If you do not agree, please do not use our services.
Our services are provided by licensed Psychiatric Mental Health Nurse Practitioners (PMHNPs) offering telehealth-based mental health care.
2. Services Provided
We offer virtual mental health care, including:
Psychiatric evaluations and medication management
Telehealth therapy and mental health support
Diagnosis and treatment of mental health conditions
Prescriptions (including controlled substances, where permitted by law)
All appointments are conducted via HIPAA-compliant telehealth platforms.
3. Eligibility for Services
To receive care, you must:
Be 18 years or older (or have parental/guardian consent for minors).
Be located in a state where our providers are licensed to practice.
Have a stable internet connection for video appointments.
Provide accurate and up-to-date personal and medical information.
We reserve the right to refuse service if a patient provides false information or is deemed inappropriate for telehealth services.
4. Payment, Insurance, and Cancellations
A. Insurance and Out-of-Pocket Payments
We accept most major insurance plans through Headway.
Out-of-pocket sessions cost $100 per evaluation.
Superbills are available upon request for out-of-network reimbursement.
B. Payment Methods
Credit/debit cards, FSA/HSA cards are accepted.
All payments are due at the time of service.
C. No-Show and Cancellation Policy
Cancellations within 24 hours of your appointment may be subject to a $50 late cancellation fee.
No-shows without notice may be charged the full session fee.
Exceptions may be made for emergencies.
5. Telehealth Disclaimers
Telehealth may not be appropriate for all conditions. If a higher level of care is needed, we may refer you to in-person providers.
We do not provide emergency services. If you are experiencing a crisis, call 911 or go to the nearest emergency room.
Controlled substance prescriptions are subject to state and federal laws and may require periodic in-person evaluations.
6. Confidentiality and Privacy
Your medical records are protected under HIPAA and remain confidential.
We will not share your information without your consent, except as required by law (e.g., mandated reporting, legal orders).
Secure, HIPAA-compliant telehealth platforms are used to protect your data.

7. User Responsibilities
By using our services, you agree to:
Provide honest and accurate medical information.
Use a private space for telehealth sessions to ensure confidentiality.
Follow provider recommendations for treatment and medication.
Not record or distribute telehealth sessions without consent.
Failure to follow these terms may result in termination of services.
8. Limitation of Liability
Elevate Mental Health Services is not responsible for technical issues (e.g., internet disruptions) that prevent a telehealth session.
We do not guarantee treatment outcomesโmental health care effectiveness varies per individual.
We are not liable for decisions made based on telehealth consultations.
9. Changes to Terms of Service
We may update these Terms of Service periodically. Any changes will be posted with a revised effective date.
Continued use of our services after updates constitutes agreement with the revised terms.
